This treatment is performed in the office setting. Using off-the-shelf products that are readily available, the procedure can take anywhere from 10-30 minutes.

In Office Injectables
Injectable fillers have been used for over 20 years. They are used in dermatology and plastic surgery and are now often used in the foot. This is performed in the office with little downtime and is a quick fix for foot pain that can keep you out of surgery. Whether you want to wear high heels, walk barefoot, or exercise, this treatment is a great option performed by our doctors in Boca Raton and Boynton Beach. Currently, we used RadiesseⓇ, SculptraⓇ, and Juvederm VolumaⓇ. Walk-in with pain and walk-out more cushioned! The injections work great on high-pressure areas causing corns and calluses.
LenevaⓇ Is the newest treatment and the first of its kind injectable fat graft used for fat pad restoration. It is an Adipose Allograft Matrix (AAM) which is a graft that when placed into the foot works to build fat tissue over the course of twelve weeks. The graft is safe to use has been shown to provide new soft tissue thickness and has improved the quality of skin conditions such as corns, calluses, and skin ulceration as seen in diabetics or neuropathy patients. Liposana is the newest fat graft that is cryopreserved and acts as a scaffold for tissue cushioning.
During your visit, if the treatment is right for you, it is performed in the office. The doctor will obtain x-rays, and perform an ultrasound to look at the area to be treated. The ultrasound is helpful to identify the extent of fat pad atrophy and show the exact location to perform the injection. Numbing medication is used on the skin and the injection is painless! The doctors do not use a needle, but use a blunt tip injector to insert the medication into the foot. Adhesive pads are placed on the foot and light pressure with walking is allowed immediately. You will be provided with complete instructions for post-procedure care. It is recommended that minimal exercise is performed for at least one week after the procedure. In more severe cases, the doctor may provide a special shoe during your healing process.

Fat Pad Surgery
If surgery is needed for other conditions such as a bunion, we will often use a graft and place it into the foot. This is a layered cushion of tissue that is placed under the skin and provides an extra layer of padding. It is anchored with a skin suture and becomes incorporated into your tissue within a few weeks. It is recommended to remain off of the foot for approximately one week after treatment.
Fat Pad Transfer
Fat transfer or autolipotransplantation is another fat pad cushioning procedure. We work with our team of plastic surgeons to take your own fat from the stomach or buttocks and it is placed into the foot. This procedure can be formed in the office or in an outpatient surgical suite.
There is a slightly longer recovery time with the procedure due to the liposuction site and the foot healing, however, you can walk with a special shoe immediately after the procedure. We have found this procedure lasts the longest and touch ups can be injected at a later date.
On occasion, we will mix the fat with amnion stem cell or PRP (protein rich plasma) to stimulate the fat and lay down more padding tissue. This procedure is often used when larger volumes of fat are needed in the foot. By transferring fat from one body part to another, we can cushion the bones of the foot and provide a more comfortable pillow to walk on.

What Should I Do After My Injection?
Adhesive pads are placed on the foot, and light pressure with walking is allowed immediately. You may have some slight discomfort the following day or two, and avoiding any inflammatory medication such as Advil, Ibuprofen, Motrin, Aleve, Mobic, etc is advised. We want the body to have an inflammatory response as this will begin the regeneration tissue process called biostimulation. If you do have discomfort, it is recommended that you take Tylenol or acetaminophen. You will be provided with complete instructions for post-procedure care. It is recommended that minimal exercise is performed for at least one week after the procedure. The doctor may provide a special shoe during your healing process in more severe cases.
How Long Does The Injection Last?
Although we cannot predict how long the material lasts nor guarantee how much relief you will get, we can state our research findings. Injection of hyaluronic acid with the use of Juvederm showed the effects of the material, providing relief for over one year. Leneva patients were followed for three months with 75% improvement of their symptoms and improvement in their quality of life. Dr. Schoenhaus has seen the effects last anywhere from one to three years. Most patients return for another injection when their symptoms wear off. Performing the booster injections is safe and effective.

How Long Does It Take To Feel The Benefit Of Fat Pad Restoration?
Immediately after the injection, you will feel a “plump” filled area. This is the immediate fill of the injection material into the foot. After the first week, this plump feeling will decrease. By the second week post injection your pain should significantly improve.
The tissue has now been incorporated into your tissue, and the regeneration process has started. Your optimal improvement is approximately twelve weeks post-injection. It is at this time you should come into the office for your follow-up appointment (medical travel patients have a televisit). If you have reached significant improvement and you feel that you are back to a quality of life that you are pleased with, the doctor will provide additional instructions for follow-up and care. If you decide you would like more improvement, the doctor may suggest a touch-up injection. This may be needed in approximately 5% of patients.
The benefit of fat pad restoration is not a guaranteed prediction, and results may vary. Factors contributing to the success of the injections include the severity of the condition and other factors, such as painful neuropathy.

Fat Pad Augmentation Post Procedure Facts and Instructions
The results of fat pad augmentation are not immediate. After your first treatment visit, you may feel the increased cushioned effect because of the filler injected. A few days following the treatment, when the swelling goes down and the solution is absorbed by your body, the area may look and feel as it did before your treatment.
Fat pad augmentation takes approximately eight weeks to feel the maximum benefit. We will see you one week after your treatment session to make sure everything is healing properly. Please note that you may need a touch-up six weeks after your initial treatment.
What to do and expect after Fat Pad Augmentation treatment:
- Discomfort: The area might be tender to the touch after treatment, and an uncomfortable sensation may be present for a few days. You may also feel some inflammation in this area. This is normal. However, if the area is red and/ or warm, call the office. You can take Tylenol for this. Do not take Motrin or any anti-inflammatory medication.
- Ice and elevate the treatment sight as much as possible for at least a week
- Be sure to wear comfortable, supportive shoes such as sneakers.
- Wear your pads or orthotics, as recommended by the doctor. Remove pads to shower and reapply after the area is dry. Change the pads every 2-3 days. If you need more pads, please contact the office.
- If you were given a procedure shoe, please wear it as advised by the doctor.
- You may return to physical activity 2 weeks after your procedure unless instructed otherwise.
- The effects of fat pad augmentation vary from person and person. We cannot predict outcomes prior to the procedure, but we can help you get the most out of the procedure by following the recommended guidelines.
